What is Phimosis?



The foreskin is a protective layer that nature has bestowed upon the penis from the beginning of time. With this, the glans penis is safeguarded.

3. Ideally all men should be able to retract the foreskin behind the glans.
4. The most the most prominent part of the glans is called as corona.

You should have no trouble pulling the foreskin back. You should be able to easily pull the penis back a few millimetres behind it, no matter how round or firm it is or whether it is being held up. It seems that they can’t do this with some groups of guys. That’s what phimosis means.


What are the causes?



What causes this after birth?


Babies would not be able to retract the foreskin. After a few years, the baby can gradually pull back the foreskin. But you need to understand. This is a natural phenomenon. So no need to worry!

But occasionally, in certain situations,

Phimosis can also result from forcing the foreskin back before it is ready. A fibrous scar may result from this. In the future, this might stop the foreskin from retracting.

What are the pathological causes of individuals’ inability to retract their foreskin?



• The first and foremost is repeated infections in diabetic patients,
• Repeated trauma, In these situations, there is repeated scarring of the foreskin.
• You are not able to retract the retract the foreskin.



What are the grades or types of stages of phimosis?




There are three classifications for phimosis: mild, moderate, and severe, ranging from 0 to 5.
However, the most common and accepted classification system is one to four, which is graded from one to four.



Why do we need to classify these this problem?



The classification assists us in identifying the group of patients who will consistently receive medication and local ointments, as well as the other group of patients who would not benefit from these forms of medical care and require a surgical procedure.

In Stage or Grade 1 is when you are able to retract the all these grades, all these stages are in erect state.

• In Stage 1 is when you are able to retract the prepuce behind the corona and there is some stretch in the erect state.
• Stage-2 is that you are not able to fully retract it beyond the corona.
• Stage-3 is when you are able to retract it and are able to see only the meatus. Meatus is the portion of the of the glands where from the urine comes out the outer opening.
• Stage 4 is when you are not able to see even that.



Why is it necessary to treat these problems surgically in a specific subset of patients?



1. One is that it can cause infection because there is a natural secretion of smegma that is naturally occurring, and if you are not able to retract and clean it on a regular basis, this is a milieu that causes repeated infections.
2. Second reason, and if these repeated infections keep on occurring for a long year or a long period of time, they can cause penile carcinomas. Additionally, a persistent odor may arise, causing significant discomfort for both you and your partner.
3. The third reason is that due to the extreme pain associated with sexual intercourse or masturbation, people tend to avoid such activities, which can lead to erectile dysfunction.Therefore, erectile dysfunction is one of the most common problems that can be easily treated.



What are the treatments available for stage 1 and Stage 2?



We generally treat with local steroid ointments, followed by stretching exercises and the use of specific rings.

However, as a surgeon, I am not a strong supporter of rings and forceful stretching because, as soon as you stop the stretching or apply the rings, scarring occurs, putting you back in the same position and making it impossible to retract permanently.



When does this surgery help in stage 3 and 4?



Is circumcision the only solution?

No.

There are other ways of treating this problem. Circumcision is not the only solution. There are other treatment options such as prepusoplasty, dorsal slit, and circumcision.
